2022-2023 Sixth Grade Supply List

Supplies Needed:

 1x Wired Earbuds

Note: Please label the items with your student's name and place them in a Ziploc bag. Rolling backpacks are allowed.

Medication Kit:

The medication kit will remain in the front office the entire year. On the first day of school, if needed, please send the following items in a Ziploc bag labeled with your child’s name and give it to your teacher:

 Any medication your child may need with specific instructions for use

 If you are including medication, please complete and sign this required form and include it in the Ziploc bag
