Please note that the following requirements must be met before volunteering in the classroom:

Personal Health Statement
Proof of MMR vaccine
Proof of Tdap vaccine
Proof of current flu vaccine (within the year) or decline letter.
Volunteer - SCCPHD TB Risk Assessment form or skin test (no more than 1 year prior to start date)
